Hemoptysis can occur in various conditions, but is mostly seen in lung diseases such as bronchiectasis, pulmonary tuberculosis, and lung cancer. However, there are some uncommon conditions that can cause hemoptysis. Several reports have suggested that patients with pheochromocytoma can present with hemoptysis. Until now, there has been no report in South Korea of a patient with pheochromocytoma initially presenting with massive hemoptysis. We report the case of a 22-year-old male patient with massive hemoptysis and circulatory collapse, who was subsequently diagnosed as having a pheochromocytoma. This case suggests that clinicians should be aware of the possibility of pheochromocytoma in patients who have hemoptysis, but no history of previous lung disease. (Korean J Med 74:S189-S193, 2008)
